This article describes the social work service delivery model in the hospital system, the role of social workers in supporting the hospital system in Australia through desk review and analysis methods. The research results show that hospital social workers in Australia provide direct services to minimize the impact of illness on the patient's hospitalization and discharged from hospital. Accordingly, social work is considered an essential and continuous service in the service chain provided by the hospital system. Based on Australian practice experience, the article offers suggestions for Vietnam to refer to in the process of building and developing professional social work teams in hospitals .
